Information or advice about your wellbeing – or how you can help somebody else.
If you contact North East Wales Mind, engage in our services or request information from us, we may obtain information about you. This statement explains how we look after that information and what we do with it.
We have a duty to process your data legally and fairly. We must also ensure that the data we hold is accurate, adequate, relevant and not excessive.
Normally the only information we hold comes directly from you. Whenever we collect information from you, we will make it clear what information is required in order to provide you with support or services. You do not have to provide us with any additional information unless you choose to.
We store your information securely on North East Wales Mind’s secure database, or on paper records. We restrict access to these records only to those who have a need to know, and we train our staff in handling the information securely and confidentially.
In some cases we work together with other organisations to provide services to you. These are CAIS (as partners in the Parabl consortium) and FLVC (as partners in Step up Step Out). If you have made an enquiry or are using the services of Parabl or Step-up Step-out we will pass your details on to the CAIS or FLVC worker involved in providing your service. Otherwise we will not pass you information to other organisations without your consent.
We would also like to contact you in future to tell you about other services we provide and ways in which you might like to support us. We will always ask for your agreement to be contacted in this way. You can also contact us directly at any time to tell us not to send you any future material.
You have the right to a copy of all the information we hold about you. To obtain a copy, either ask for an application form to be sent to you, or write to the Data Protection Officer at North East Wales Mind, The Wellbeing Centre, 23b Chester Street, Mold CH7 1EG. There is no charge to obtain this information and we aim to reply as promptly as we can and, in any case, within the legal maximum of one month.
We will only keep your data on our systems for a reasonable length of time, that is, no longer than 3 years after your engagement with us ends unless the matter is covered by the Limitation Act 1980 then the relevant time limits apply.
You have the right to have the data we hold on you erased from our records at any time.
If you have given your consent for us to contact you about services, surveys etc. you may withdraw this consent at any time.
